CONFIRMATION PRE-PROGRAM INTERVIEW
Confirmation Pre-Program Interviews MUST occur in person at the Parish Pastoral Center. Please read the important information below.
PLEASE NOTE: If you do not have a copy of the Baptismal Certificate, call and request one from the Church of Baptism ASAP. Please plan to reschedule if the Baptismal Certificate or Online Registration Form have not been submitted.
BEFORE THE SCHEDULED INTERVIEW DATE:

Baptismal Certificate must be submitted

Online Registration Form and One Page Paper must be submitted BEFORE your scheduled interview

Make sure to bring your medical liability and communication release form. Click here to print the form. (It is 2 pages)
ON THE SCHEDULED INTERVIEW DATE:

Teen should BRING THEIR CELL PHONE (we will make sure they are able to receive texts from our communication system)

Parent and teen need to be present

Interview will last approximately 20min

bring the $50 Confirmation Fee, if not previously paid.
AFTER SIGNING UP FOR THE PRE-PROGRAM INTERVIEW, REMEMBER TO SEND THE BAPTISMAL CERTIFICATE TO
